NYC Social Worker Launches HealthcareCheck — White-Label Patient Navigation for Community Health Centers at $299/mo
The HIPAA-compliant platform integrates 8 validated clinical instruments, FHIR R4 payer connections, and AI-powered referral drafting across 691,000+ community resources — deploying in days, not months, at a fraction of enterprise health IT costs.
Mental Wealth Solutions PLLC today announced the public launch of HealthcareCheck, a white-label patient navigation platform designed for Federally Qualified Health Centers (FQHCs), Certified Community Behavioral Health Clinics (CCBHCs), dialysis centers, and VA-affiliated programs. The platform enables healthcare providers to deploy a fully branded, HIPAA-compliant patient screening and care coordination app under their own name, logo, and domain — without writing code or hiring a development team.
“I built this because I’ve been the social worker sitting across from a patient who needs housing, transportation, food assistance, and mental health support — all before they can even start treatment,” said Sexton. “Enterprise platforms cost $150,000 or more to implement. Community health centers can’t afford that. HealthcareCheck gives them the same capabilities for less than the cost of a single avoided emergency room visit.”
8 Validated Clinical Instruments
Built directly into the patient experience — no add-ons, no extra fees.
FHIR R4 Payer Integration
Direct OAuth 2.0 / PKCE connections to 6 major insurers.

Sexton served as Director of Social Work at Atlantic Dialysis Management, one of New York City’s largest independent dialysis providers, where he built the social work department from the ground up across 13 dialysis clinics. In that role, he managed CMS and NYSDOH compliance, improved ESRD Quality Incentive Program (QIP) star ratings, increased transplant listing rates, created the organization’s first centralized patient database, supervised clinical social workers, and trained staff in ACT, CBT, and Motivational Interviewing. He also led a facility-wide Covid-19 vaccination initiative that achieved a 95%+ patient vaccination rate.
His clinical career spans high-acuity populations across behavioral health, substance abuse, forensic services, HIV, and disaster recovery:
- Arms AcresOutpatient substance abuse therapist. Designed an 8-week ACT-based group therapy program for court-ordered, parole, and probation clients.
- Central Nassau GuidanceForensic Assertive Community Treatment (ACT) team clinician. Multi-disciplinary community-based outreach that reduced psychiatric hospitalizations among high-risk individuals.
- NADAPProgram Coordinator for the NYS Nassau-Suffolk HIV Employment Initiative — a state grant-funded program with monthly compliance reporting to the NYS Office of Temporary and Disability Assistance.
- Salvation Army EDSSuperstorm Sandy disaster case management. Navigated federal and state disaster assistance programs, conducted housing needs assessments, and managed home repair projects for displaced families.
“HealthcareCheck exists because Sexton spent five years inside the dialysis system watching social workers track SDOH needs on paper, lose referrals in spreadsheets, and struggle to close the loop on patient navigation — and decided to build the tool he wished he had.”
